Care Coordinator #8006

Hillside Children’s Center has an opening for a Care Coordinator in Lockport, NY.  This is a full time, 40 hour position, Monday – Friday.

The Care Coordinator II has primary responsibility for the child’s health care coordination and administration of the Individual Health Plan. The Care Coordinator II (CC II) provides the service of Health Care Integration, the OCFS Bridges to Health Waiver service through which the Individualized Health Plan (IHP) is created and managed. The Health Care Integrator will assess the need for services, develop and update the IHP, link the child and family/care giver to the identified services. They will also consult with the child and care giver on the appropriate provision of services, stabilize the environment for the child and care giver, and advocate for the child’s needs, coordinate team meetings, coordinate the constellation of services and providers to meet specific needs of each enrolled child; and participate in overall quality management of B2H Waiver Program.

Qualifications

Required: The minimum qualifications for a CC II are a Bachelor’s degree in Social Work, Psychology, or other human services related field and four years of experience providing service coordination. This position requires an independent self-starter who is able to work from home. NYS Driver’s License required. Candidates must meet the agencies driving and insurance standards.

Preferred: Master’s degree in social work, psychology, or other related field, or to be licensed as a qualified Health Care Practitioner, a Registered Nurse, or a Special Education teacher and a minimum of one year of experience providing service coordination and information, linkages, and referrals for community-based services to children with special needs, individuals with disabilities, or seniors.
