Hillside Children’s Center has an opening for a Care Coordinator for our Waiver Services program in Lyons, NY.   This is a Full Time - 40 hour position, Monday - Friday 9am to 6pm.

 The Care Coordinator (CC ) is responsible for the delivery of quality psycho-social care to the youth and families that Hillside serves. The Care Coordinator provides clinical support and/or therapeutic care as assigned. Essential functions of the CC role include, but are not limited to the following:

• Working as a team member in assessing a youth’s needs, responsibility for developing, assessing and modifying the individual service plan in collaboration with the youth, family, and multi-disciplinary treatment team.
• Actively involved in the implementation of a strength-based service plan, including site/home visits and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results, and communicating with the multi-disciplinary treatment team.
• Utilizes experience, education, age-specific knowledge, and cultural competence, in order to assess and support youth and families to achieve positive and successful outcomes.
• Routinely and successfully uses problem-solving skills with own self and with others.
• Ensures HFA policies, procedures, and all applicable governmental laws, regulations, and guidelines are followed.
• Makes decisions, and has authority to implement decisions in conjunction within scope of responsibility. This position will also assist the teams in ensuring services are delivered and compliant with regulating funders organizations.

Qualifications

 

Candidates must have a Master’s Degree in Social Work or a relevant Human Services field, along with 2 years of relevant work experience or Bachelor’s Degree or a relevant Human Services field, along with 4 years of relevant work experience. Relevant Work experience is defined as providing direct services for children in one or more of the following primary diagnoses: mental illness, mental retardation, alcoholism, chemical dependency, or substance abuse. Candidates must have experience supervising other team members, primarily other care coordinators and must regularly perform the care coordinator functions with significant independence and expertise, and be relied upon as a resource to guide team performance. Candidates must have a valid New York State Driver’s License and must meet agency driving and insurance standards.
